Whether that’s related to drugs & alcohol, bereavement, mental health, financial or housing (and more), Referral Partners can create and edit ‘hyper-local’ service listings relevant to their communities, giving them utmost flexibility.

No Patient Data required.
My Community Referral is designed for patients across Primary Care who need additional help from the community. All part of supporting the NHS early-intervention and prevention strategy.
Works in partnership with Link Workers & Community Navigators.
If a patient has more complex or additional needs a Community Referral ID can be created, referring the patient into a Link Worker. The system ensures Primary Care support teams are also aware of all local support and can track the patients journey.
